Sharp Home Care now offers
psychiatric home care services to adult and senior
patients living in San Diego County. The in-home
services are brought to patients through a
multidisciplinary team of professionals specializing in
the treatment of people with mental health issues.
Psychiatric home care services
address the unique needs of each patient, whether the
individual:

Is transitioning from
inpatient treatment to home

Is having significant mental
changes because of a medical episode or change of
condition

Is unable to leave his or her
home or is unsafe to attend psychiatry appointments

Is lacking knowledge about
his or her medications, treatment or disease process

Is in need of physical or
occupational therapy to address safety and self-care
issues
-
Has a caregiver who needs
training to address current medical or mental status
The Sharp team assesses
individual patient needs and works closely to develop
effective treatment plans. Team members include:

Physical Therapists

Home Health Aides
The medical director is a
psychiatrist and available for team conferences and
physician consultation as directed by the patient’s
attending physician.

To Make a Referral
Sharp psychiatric home care
services can be given to qualified patients at home, in
a group home or at an assisted living facility, also
known as “board and cares” or RCFE’s.
Referrals can be made by anyone,
but require orders from a medical or psychiatric
physician.

Psychiatric home care is just one
aspect of the continuum of care available from Sharp
Behavioral Health Services, a Sharp Center of
Excellence. Comprehensive services also include acute
inpatient care to partial hospitalization and outpatient
services for seniors, adults and children.
Medicare covers necessary
psychiatric home care. HMO and PPO insurance companies
may also provide coverage and private pay arrangements
can be made. Sharp Home Care can assist in exploring
covered benefits and financial resources as needed.
Sharp Home Care is a
Medicare-certified home health agency licensed under
Sharp Memorial Hospital and accredited by the Joint
Commission on Accreditation of Health Care Organizations
(JCAHO). Sharp also holds membership with the California
Association for Health Services at Home and the San
Diego & Imperial County Regional Home Care Council.
